I having the same issue with my 50 Inch, I have tried all fixes, and it doesn't happen everywhere but when I Record a show most times the Audio is out of Sync with the video, It is fine if I watch it on another TV or my Ipad but not on the Insigna the audio doesn't match with the video on shows.
My TV info is as follows
Model: INSIGNA_NS-50DF710NA21
Serial # {removed per forum guidelines}
I have tried the factory reset, updating the software and changing the audio settings to PCM still doesn't fix the issue.
