Well by doubling up I think she means using the old fashioned "RCA" style audio out connectors which are typically red and white, left and right.........I mistakenly replied above that I could not do that howver my AV home theater /surround sound receiver DOES have thoe connections as does this Toshiba FireTV. I did a test last night. I switched to live antenna fed TV and hit pause, (yes you can do that with the tv just like cable/satellite DVR) and left it on pause for about 4 min. After hitting pause again to resume, LOUD screeching noise.......so this means that the problem/bug is NOT just with the Amazon Fire interface alone but the TV in general. Doubling up would NOT likely solve this and this HAS to be a firmware/software BUG. Since it obviously affects regular tv and not just Amazon Fire, I'm thinking that Toshiba will need to release a firmware update..........then again, this TV is Amazon exclusive even though it's a Toshiba, and sold exclusively by Amazon and Best Buy................ perhaps Amazon would do the update but it could also fall on Toshiba. Can someone help us with this?

Wanted to let you know I have a similar problem with volume on my 55" Fire TV. Mine happens when I turn off the TV while playing a stream, then turn it back on again. For me, the volume ends up louder than when I turned it off, and the only way to fix it it "restart" the TV (which fully reboots it). Since the way the Fire TV "turns off" is not really turning off but existing the app mid-stream and go back to the home screen when you turn it on... I have a feeling this is more related than it may sound.
My post is here
Hopefully I'll never have the volume pushed to max like you did...

I have a smaller 43" version of this Toshiba FireTV edition. Glad to see that I am not the only one with severe audio issue of connection to my reliable Yamaha RX V779. I tried to use HDMI ARC and there as no success with that, to switch the audio from the TV to AVR. Then I fooled around and one time was able to get the sound out through Optical connection, but now it is not working.
I think the audio output is severely messed up...and I am not sure if I can get this fixed...will consider returning this piece of junk if I still can't get the TV audio output to my AVR.

I am planning to return the Toshiba 43" Fire TV as there is definitely a built in problem with audio signals not outputting through the HDMI ARC as well as the optical digital. The TV looks great but defeats the purpose of using my current Yamaha RX V779.

I found this "Smart TV" is not really capable of ARC utilizing just HDMI Cables. That is why it is been said "doubling up" is the solution? I rather not have to use two methods for audio. Perhaps it is because the "Toshiba Fire TV" has only two channels for audio and thus-ly not capable of true ARC? I mean thats all well and good, but for me, my DVD requires the optical port on my Yonko Receiver. Bummer is they are both supposed to be capable of ARC which would resolve the audio issue altogether-NOT. Lol
I'd say stop going to a novice retailer that refers everyone to "Geek Squad" when ever their product is inferior to begin with......."Just my 2-cents"
