Yet to receive $25 bonus card from Cardmember Appreciation offier

I received and activated the spend $250 and get $25 card back that was mailed out on Oct 1st, and I made my purchase on the 10th, the last day of the promotion. I have receved points from the purchase but never received my bonus card from the promotion. It said that the card would be emailed between 3-4 weeks and it is going on 6 weeks now. It appears that there are a few people with the same issue so it doesn't seem to be just me.
