Birthday reward disappeared

I got a code for January for birthday rewards, 10% off. I tested it out on the item I wanted to buy and it worked. When I went back to actully use it, it was gone. My account now statest hat I have no rewards available. I think what may have happened is that my wife accidentally registered under the same email address and maybe it invalidated my offer. However, all of my purchase info is still appearing. Is there a way for me to get this back? In the future, paper rewards would be preferable. Or, an email with the code would be even better.
