I'm having the exact same problem. Were you able to get it resolved? I have repeatedly contacted Sprint and they indicate that Best Buy has to resolve the problem which is very confusing since Sprint has my phones but has yet to give me credit for them.

Was your phone purchased in one of our local Best Buy stores? If so, the store will be best suited to try to get this fixed with Sprint if you were eligible for the promotion.
