Custom Certificate of Insurance

I just placed an appliance order for delivery and installation.  My condo requires a custom certificate of insurance (Certificate Holder and Additional Insureds).  Please let me know how I can go about securing this.

Thank you bobberuchi.  Unfortunately, this is the generic version and my condonimium rules require the property management company and the condo unit holder specifically listed on the COI.

My apologies I find it perplexing.

 

Many thousands of customers  every year use the generic certificate.  And every so often there is an office or property that has an issue with it.
