I have reached out to multiple contacts within your customer support team to process this custom COI and each COI we have received has been incorrect. We have a sample COI from our association on how the COI needs to be filled out exactly. I have asked customer service if there is a way I can email them this COI Sample so that it is done properly and they said there is no way. I need to get this done ASAP and need to send this sample to somebody so that I can submit to my association before our TV delivery. Please let me know if there is someone I can send this to.

Welcome to our forums! As someone who’s helped my parents with deliveries to their condo just a few months ago, I know how important it is to get your COI not only as soon as possible, but completely accurate as well.
