Yet Another Failure to Receive Pre-Order Credit

I have tried other methods for getting the $100 for 5 pre-order credit and have not had any luck so I figured I would try here. My last purchase was Zelda on 11/20 but never got confirmation that the bonus was fullfilled.

 

Any help would be greatly appreciated.
